HPRP Family Collaborative
The HPRP Family Collaborative is a joint effort led by Families In Transition of Santa Cruz County. This project will provide assistance to
struggling families throughout Santa Cruz County in 2010 and 2011. Six Santa Cruz County organizations are working together to provide temporary support for rent,
security deposits, and utilities to local families to help them through the economic crisis. Please visit the new HPRP site

FIT Referral Form
Agencies wishing to refer a client for any of our general services can download a copy of our
Official Client Referral Form. After completing the Referral Form, please email it as an attachment to
info@fitsantacruz.org. We will then contact elligible clients once they have been added to our waiting list.

Welcome
Welcome to Families In Transition of Santa Cruz County. The goal of Families In Transition is to provide high quality supportive & housing services to struggling families throughout Santa Cruz County.
Families In Transition works closely with many community partners to ensure that families are able to access all appropriate resources and opportunities for success and stability. In addition to linkages with community programs and on-going case management services Families In Transition provides a variety of housing strategies to meet the individual needs of homeless and at-risk families with children throughout Santa Cruz County.
In most cases Families In Transition provides a short-term housing subsidy to families in scattered sites throughout the community. The goal is for the family to obtain necessary rental assistance and social services tailored to their individual circumstances while living in stable, private market housing that they will be able to maintain on their own at the completion of Families in Transition services.
Since 1992 Families In Transition has been preventing and ending homelessness for local families with children. Over one thousand families have received services that have enabled them to provide a home and greater stability for their children.
